How to apply for the My Number Health Insurance Card Registration Campaign via PepUp

 ❶ 　Click on Applications (各種申請) 

 ➋ 　Select Insurance Card Registration Campaign for My Number Card

 ❸ 　Select your name  and click “New Application"(新規申請)をクリック

 ❹ 　Enter "1"、"application date", Upload to Pepup the screenshot or
          downloaded file of “Medical Insurance Eligibility Information” from
          Myna Portal, check the “Consent to Personal Information” checkbox,
         and click “Confirm Contents”.

 ❺ 　Confirm the contents and click “Apply"(申請する)
