Purpose of the Check
The insurance premiums paid by an insured person (employee) and the employee's company are used as the funds for the operation of this health insurance society, and as such, the funds must be used appropriately for insured persons and their dependents.
To ensure this, the certification of the dependents of insured persons must be carried out strictly.
What is more, we are required by law and Ministry of Health, Labour and Welfare notification to carry out an annual check to confirm whether a person continues to meet conditions for qualification as a dependent even after certification as a dependent.
We ask for your cooperation as this is a confirmation measure that needs to be taken to ensure that your insurance premiums will be used appropriately.

Important Notes
If, as a result of this qualification check, a dependent person is determined to be outside of the conditions for certification as a dependent, you will need to follow the procedures to remove that person as a dependent.
In such cases, please fill out a "Notification of Health Insurance Dependent (Change)" application form and submit it and the health insurance card for the person in question (along with a Certificate of Application of Maximum Copayment Amount, Elderly Recipient Certificate and Certificate Issued for Specific Disease Treatment, if relevant) to the person in charge of human resources at your company.
Please keep in mind that if you fail to submit the Health Insurance - Dependent Confirmation Statement by the deadline, under Paragraph 9 Article 50 of the Enforcement Rules of the National Health Insurance Act - which states that the health insurance cards(or Health Insurance Eligibility Certificate) of insured persons who fail to receive validation or renewal at the time of validation or renewal will be invalidated - there are cases in which the qualification as a dependent will be eliminated for the relevant person.
If a person is removed as a dependent, a bill will be sent to the insured person at a later date for any medical expenses, medical checkup fees and other costs that may have been borne by this health insurance society after the date of removal as a dependent.
