I would like to have a health checkup. How can I make an appointment?

The following outlines the process from registering for the service and completing the initial setup to booking your health checkup.

 

1) Access the website and begin registration
Go to the i-Wellness website: https://i-wellness-p.com

 

*If you would like to continue in English, select “English” under Language in the upper-right corner before proceeding with the steps below.

 

First, click “Make a registration.” under “Registration of use.”
Please note that last year’s login credentials have been reset. Please register again for the current fiscal year.

 

2) Agree to the Terms of Use
Scroll to the bottom of the Terms of Use page to review the content, then click “Agree”

 

3) Enter your basic information
On the website registration page, enter your customer number, name, gender, date of birth, email address, security question, and answer.

 

4) Complete your registration
After entering the required information, click “Register.” A registration completion email will be sent to the email address you provided.

 

5) Log in for the first time
Use the temporary password provided in the completion email and your email address to log in and proceed with booking your health checkup.
